If you are transgender or non-binary, the idea of starting counselling might come with an extra layer of hesitation. Not because you do not want support — but because past experiences may have taught you to be cautious about who you trust with the most personal parts of your life.

Perhaps you have had to explain your identity before someone would take you seriously. Perhaps a previous professional used the wrong name, asked intrusive questions, or treated your gender as the thing that needed fixing rather than listening to what you actually came for. Perhaps you have simply never found a therapist where you could relax enough to talk about what really matters — without first having to establish that who you are is not up for debate.

That exhaustion is real. And it is one of the most common reasons trans and non-binary people delay seeking support, even when they know it could help.

At Hope Therapy, your identity is not the starting point of a negotiation. It is respected from the first conversation. Both are equally valid reasons to reach out.

We work with people at every point in their experience. You do not need to have a diagnosis, a referral, or a clear sense of what you want to work on. You just need to feel that talking to someone — someone who already understands — could help.

What you might bring to counselling

Trans and non-binary people come to therapy for all the reasons anyone does — and for some that are more specific to the experience of being gender-diverse in a world that often is not set up for you. You do not need to be in crisis. You do not need to have a clear goal. You just need to feel that talking to someone could help.

  • Processing difficult feelings around your body, whether or not you are pursuing medical transition
  • Grief — for time lost, for relationships that changed, for the version of your life you were expected to live
  • Building confidence, self-acceptance, or reconnecting with parts of yourself that were set aside

You do not need to be experiencing gender dysphoria to seek counselling. Many people come because they want a space to think, to process, or to be fully themselves — without filtering.

How affirming counselling works — and what it is not

Gender-affirming counselling means your therapist works with you from a position that respects your identity as it is. Your gender is not treated as a hypothesis to be tested, a phase to be waited out, or a problem to be solved. It is the ground you stand on — and therapy builds from there.

This matters because the alternative — counselling that questions, pathologises, or subtly steers you toward a different conclusion about who you are — can cause real harm. Hope Therapy is clear on this: we do not practise or support any form of conversion therapy. This is our position as an organisation, and it is the ethical standard to which all our therapists are held, in line with the NCPS Code of Ethics and the BACP prohibition on conversion practices.

What affirming counselling does offer is a relationship where you can bring whatever is on your mind — gender-related or not — and know that it will be met with understanding. Your therapist may help you explore patterns of thought or feeling that are causing distress, develop strategies for managing difficult situations, or simply hold a space where you do not have to perform or explain yourself.

For some people, that means working through anxiety that has built up over years of concealment or hypervigilance. For others, it means processing grief — for the childhood they did not get to have, for relationships that changed, or for the version of themselves they were expected to be. Some people come because they want help navigating a practical situation — a workplace transition, a difficult conversation with family, or the waiting and uncertainty that can come with accessing medical services.

Whatever you bring, the work happens on your terms. There is no agenda beyond what matters to you.

Sessions are confidential. There are limited circumstances where confidentiality may need to be adjusted — for example, if there is a serious risk of harm to you or someone else — and your therapist will explain these clearly at the outset.

Hope Therapy provides counselling support for the emotional and psychological aspects of being trans or non-binary. We do not provide medical assessments, hormone prescriptions, or referrals for gender-affirming medical treatment. If you are looking for those services, your GP or a specialist gender identity clinic would be the appropriate starting point.

Therapist availability changes from week to week, so rather than asking you to choose from a directory, we take time during your free 15-minute consultation to understand what you are looking for — and then match you with a therapist suited to your needs.

During the consultation, we will ask about:

  • What you would like the work to focus on, and any specific concerns you would like support with
  • Whether you would prefer face-to-face counselling, online sessions, or a combination of the two
  • Any preferences around therapy approach (counselling, CBT, EMDR, hypnotherapy, mindfulness, ACT, compassion focused therapy and others)
  • Day and time availability that works around your life
  • Any specialisms that matter to you — for example LGBTQIA+ affirming therapy, neurodiversity-affirming support, or particular life experiences
  • Practical preferences — for example therapist gender, age range, or shared lived experience where that matters to you

All therapists we work with are qualified and registered with appropriate UK professional bodies, and we will confirm the most suitable options with you before any sessions begin.

Frequently asked questions

Do I need to be “far enough along” in my transition to seek counselling?

No. There is no threshold you need to meet. People come to counselling at every stage — questioning, early in their exploration, mid-transition, years after transitioning, or choosing not to transition at all. Wherever you are is a valid place to start.

Will my therapist understand trans and non-binary experiences?

We match you with a therapist who has experience working with gender-diverse clients. During your free consultation, we ask about your preferences — including whether you would like a therapist with specific LGBTQIA+ experience, lived experience, or a particular therapeutic approach. If the match does not feel right, we will find someone else at no extra cost.

Is this gender-affirming counselling?

Yes. Hope Therapy takes an affirming position — your gender identity is respected, not questioned. We do not practise or support any form of conversion therapy. Can I access trans and non-binary counselling online?

Yes. All of our therapists offer online sessions via Zoom or telephone, so you can access affirming support from anywhere in the UK. This is particularly valued by clients who live in areas where finding an affirming therapist locally can be difficult. Face-to-face sessions are also available in locations across England.

Is everything I share in counselling confidential?

Yes. Sessions are confidential in line with professional ethical standards. There are some limited exceptions — for example, where there is a serious risk of harm to you or someone else — and your therapist will explain these clearly before you begin.

How much does transgender and non-binary counselling cost?

Individual counselling starts from £65 per 50-minute session. CBT starts from £85 and EMDR from £95. We may be able to offer sessions at a reduced rate — just ask during your free 15-minute consultation. There are no hidden fees, and your therapist and exact cost are confirmed before you commit to anything.
